Any patient with a chronic, progressive or terminal medical condition who:

- Is struggling to integrate his or her medical condition
- Needs psychological, emotional, social support
- Experiences difficulties addressing practical concerns related to health issues
- Faces a change in status, condition or setting
- Repeatedly needs or will need in-patient hospital care
- Will enter or is in hospice care
